  • Communicate effectively with triage and clinical teams regarding patient status, needs, and care options

  • Support outreach and community health initiatives to improve access to healthcare for medically vulnerable populations

  • Maintain compliance with FSC policies, procedures, and applicable healthcare regulations

  • Document patient interactions and interventions accurately and promptly

Qualifications

Required:

  • Current EMT certification and experience in medical clinic or community healthcare settings

  • Strong clinical, communication, and relationship-building skills

  • Experience working with medically vulnerable populations, including individuals experiencing homelessness

  • Ability to work independently and collaboratively in a multi-cultural, outreach-focused environment

Preferred:

  • Bilingual in English and Spanish

  • Prior experience in community health, outreach, or case management

Compensation and Benefits

  • Company-paid Life & Long-Term Disability insurance

  • 401(k) with 3% employer contribution plus dollar-for-dollar match up to 2%
