Job Description
The RN Care Manager position requires a thorough understanding of clinical, financial, administrative, and psychosocial components of the healthcare continuum The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members across the continuum of care. These services are provided face to face or telephonically to facilitate the creation of and adherence to, a treatment plan and optimize patient health. In conjunction with the interdisciplinary team, this position proactively addresses potential or actual clinical and/or psychosocial needs identified through screening to ensure developmental, level of care and health status transitions are safe, seamless, and coordinated. Shriners Salt Lake City is looking for a full time RN Care Manager to join our team!
Key Responsibilities:

Collaborates by working with patients, families, caregivers, and the health care team locally and within the SC system to jointly communicate, problem solve, and share accountability for optimal outcomes

Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care

Advocates by supporting or recommending services for the protection of the patient's health safety and rights

Utilizes knowledge of Medical Necessity, quality of care, and desired outcomes to assure appropriate Resource Management
Required Qualifications:

Bachelor of Science in Nursing

Current Utah RN license
-
Case Management Certification (CMC, ACM or ANCC) within 2 years of hire
Preferred Qualifications:
-
1 year Care Management in Pediatrics
-
3 years clinical nursing experience-pediatrics preferred

Bilingual
The pay range for this position is $80,912.00 - $121,388.80. Compensation is determined based on years of relevant experience and departmental equity.
