Requirements:
<ul>

DDS or DMD degree from a recognized dental institution
</li>
<li>
Certification in Oral and Maxillofacial Surgery from an accredited program
</li>
<li>
Possession of or eligibility for an active dental license in Utah
</li>

Board certified or board eligible status
</li>
