SCHEDULE FOR OUR RESIDENT CARE COORDINATOR - PATIENT ADVOCATE:
This part-time position offers 20–25 hours per week, scheduled every Tuesday and Friday, plus one additional flexible day.
YOUR ROLE AS A RESIDENT CARE COORDINATOR - PATIENT ADVOCATE:
In this position, you'll guide residents and families through the discharge planning process, helping them feel informed and supported as they transition to the next stage of care. Each day brings meaningful opportunities to connect individuals with community resources, manage insurance-related tasks, and work closely with nurses, therapists, and administrative staff to ensure every resident receives a smooth, well-coordinated experience.
In order to do this, you'll need the following:
- Works well as part of a team
- Excellent communication and interpersonal skills
Experience in a skilled nursing/rehab facility is a plus, but not required.
